Does Functional Endoscopic Sinus Surgery Gives Long Term Result?

Functional endoscopic sinus surgery (FESS) is a surgical process to cure sinusitis and nasal polyps. The procedure uses nasal endoscopes to restore drainage of the paranasal sinuses and ventilation of the nasal cavity. With the surgery, the sinus openings inside the nose are unblocked. It allows the lining of the sinus to decrease the pressure, discharge, and pain that occurs due to blocked sinus. It is carried out in case of chronic inflammatory and infectious sinus disease.

Why FESS is done?

The root motto of FESS is to cure sinus infection and sinus pain. Sinuses are air-filled spaces in the bones of the face and head. They are connected to the nose via small openings. Sinuses often become infected and inflamed leading to sinusitis. With sinusitis, the nose become blocked, pressure or congestion is felt on the face, persistent runny nose, and mucous problems occurs. This condition also cause headache loss in sense of smell. It is difficult to diagnose sinusitis. A complete nasal examination is the key to diagnose it. Some patients respond well to medical treatment and become better with antibiotics, nose drops, sprays or tablets. But some, need a surgery to cure it. Functional endoscopic sinus surgery is indicted for Recurrent acute sinusitis, Chronic sinusitis, Nasal polyps, and Allergic rhinosinusitis It is not advised to leave sinusitis untreated as it may lead to complications with infection spreading into the nearby eye socket or into the fluid around the brain. How FESS tends to give long term results?

Functional endoscopic sinus surgery is the modern approach to surgery on the sinuses. In early times invasive operation were carried out to maximise drainage by gravity. For this purpose, large holes were made into the sinuses and almost all the lining tissue was removed. Removal of lining tissue leads to unhealthy sinuses which further worsened the sinus problem instead of curing it. FESS abolishes the traditional operation complications and tends to give a long term effective cure for sinusitis. In FESS, lining is not removed as it helps the sinuses to remain healthy. The new technique focusses on creating ventilation and drainage along the normal pathways with maximum preservation of normal structures and in particular the sinus linings. It is more of a minimal invasive process constructing the nasal pathway. During FESS, high definition cameras and very fine endoscopes which allows for a much better control and long term results.

What is the purpose of FESS procedure?

FESS is carried out under anaesthesia. A special light system and a telescope is placed inside the nose. The special light system is called endoscope. The telescope magnifies the nasal openings and helps to give you a clear view. Additional small surgical tools are used to enlarge the sinussinus can drain in to the nose. Overall, the FESS surgery takes 90-120 minutes. Most of the patients return the same day after the procedure is performed.

Risks of FESS

The most rare complications with FESS is blindness. Blindness occurs as during the process optic nerve may be harmed. However, the complications occurs rarely and the occurrence rate is very low. The other complication with FESS is leakage of Cerebrospinal fluid. Its a major issue and is generally recognized during surgery. The leakage can be repaired on stop. Other rare difficulties with FESS includes orbital hematoma and nasolacrimal duct stenosis. Many of these complications will also be related to conventional sinus surgery and never simply with FESS.

Results of FESS

FESS is a successful procedure and is effective in providing long term results. Once the sinusitis are fixed after surgery, the patient may not feel the sinusitis symptoms again. Symptoms like nasal obstruction, facial pain, runny nose etc. are often relieved immediately after the surgery. The success rate also depends upon the extent from the disease before surgery dictates the end result. In almost all the patients, the surgery makes patient life easy and comfortable post operation. But in rare cases, the patients sinusitis are not cured fully and may need another surgery to repair it off fully.

Care after the surgery

Immediately after the surgery, you may feel your nose is blocked. To help this rinse the nose with salt water. Some dressings in the form of moustache are done to heal the surgery location. Painkillers are given to relieve pain. Secondly the patient may vomit or feel nausea for the first two days. Vomiting is common and it has a coffee brown colour. But in case colour of vomit is red than consult a doctor immediately. In the initial day’s patient is advised to take clear fluids and juices. Liquid diet is recommended as the patients’ breath through the nose and may have a dry throat. To avoid dry throat give lots of fluids to the patient. Outdoor activities ae not permitted for about 4 weeks. This is because outside activities may increase the chance of infection. Only mild to moderate amount of activities are recommended in the recovery period.

Functional endoscopic sinus surgery is a successful procedure and I carried out in any ENT speciality hospital. The process is easy to carry out in a short span of time. Although, it is recommended to carry out under the consultation of a well-known surgeon. Risks associated with it are less and rare.
